Required Paper Work - PLEASE READ

This year Orem High School will require each participating athlete to have a physical Each year to participate in Athletics.

This Tuesday the 9th, we have made arrangements with Elevate to be able to provide physicals to each of our athletes. This is also a great opportunity to help support OHS athletics and cover the ever increasing Training expense we have to cover.

I would like to get all required paper work in ASAP. Please make arrangement quickly to either participate Tuesday or with your own physician.

The Physical form you need is on the left hand side under "required paperwork". In addition to this form, I will need a Tryout checklist completed be each athlete and turned into me by the end of July.
